Do I have a co-pay, how much will it be?

On your medical insurance card there should be a designated co-pay section that will explain what services require a co-pay. If you have a co-pay for outpatient physical therapy it will likely be different for everyone based on the insurance carrier. Refer to Co-Pays on the Billing page.
