What is a POC (Plan of Care)?
This document is a letter that assists in communicating with the referring physician. Once the initial evaluation is completed by your physical therapist a POC is generated from the information gathered. The POC is then sent directly to the physician and a copy is placed in your file. A more important use of this tool is to allow easier follow-up evaluations. Subsequent evaluations will be completed as POC - Re-Evaluation. This letter will also be sent directly to the physician to maintain communication and allow the physical therapist to adjust treatments as necessary.
