Policies & Procedures
Schedule Policy
New patient appointments will last approximately 45 minutes to one hour. Follow-up visits will be equivalent, however may be longer based on the individual patient needs. Upon the arrival of your first visit please notify the front desk to begin the registration process. You will be asked to sign forms pertaining to your health insurance coverage, HIPPA consent forms, and diagnostic outcome forms. In the case that you may need to cancel your appointment we ask that you please notify our office within 24 hours. Any new patient has a maximum of three 'scheduled appointment' cancellations at which point no further appointments will be scheduled.
New Patient Schedule Policy
New patients may call in to schedule appointments, be prepared with:
- Patients name, address, phone number
- Date of Birth
- Social Security number
- Area to be treated
- Referring Doctor Information (note: all patients MUST be referred by a physician)
- Emergency contact information
- Employer's Name
- Marital Status
- Surgery/Injury date (if applicable)
- Primary Insurance Information -
- Name of Insurance
- ID #
- Policy holder's name and DOB
- Group #
- Phone # to insurance company
- Secondary Insurance Information (if applicable)
In addition to the above, the following is needed for Workmans' Compensation Claims
- Adjuster's name and phone number
- Case-Manager or Rehab Nurse's name and phone number
- Claim number
- Date of Accident
- Name of Insurance Company
In addition to the above, the following is needed for Auto Accident Claims
- Adjuster's name and phone number
- Date of Accident
- Name of Auto Insurance Company (note: auto insurance MUST be the injured parties insurance - WE DO NOT BILL SECOND OR THIRD PARTY INSURANCES)
- Claim Number
Health Insurance information (please see above requirements for HI information- note: if we do not participate with the patient's health insurance, patient will be responsible for any uncovered charges - we do not wait for settlement to receive payment)
Cancellation Policy
In the case that you may need to cancel your appointment we ask that you please notify our office within 24 hours. Any new patient has a maximum of three 'scheduled appointment' cancellations at which point no further appointments will be scheduled.
Bad Weather Policy
The office is expected to open each working day. If the office is closed due to inclement weather or other office emergencies, the office manager will then change the message by 6:00am on the company answering machine. The manager will leave instructions as to when the office will re-open.
Holidays
BSR & PT will close its offices in observance of the following holidays:
- Good Friday
- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving Day and the following Friday
- Christmas Day
- New Years Day
Code of Conduct
Baltimore Sports Rehabilitation has certain responsible policies, procedures and rules which are applicable to all employees and patients to guide them on what is expected and to maintain good order and efficiency. All employees and patients are expected to conform to generally accepted standards of behavior, including observance of the Rules of Conduct listed below:
Examples of violations, but not limited to:
- Abuse or misuse of BSR's property.
- Fighting on BSR premises.
- Gambling on the premises.
- Drinking alcohol or using/selling drugs on the premises or entering the building under the influence of drugs/alcohol.
- Immoral or indecent behavior on the premises.
- Use of obscene language.
- Severe or recurring disciplinary action.
- No loitering of friends, relatives or off-duty employees is permitted.
- Horseplay - Horseplay frequently causes accidents, destruction of company property and disruption. Consequently, all horse play, such as tripping, pushing, running, or throwing objects are not permitted.
- Threats or intimidation of any kind toward another employee or patient.
Substance Abuse Policy
BSR is strongly committed to providing a safe workplace for our employees and patients. Consistent with that commitment, the company will strive to maintain a work environment that is free from the effects of alcohol and illegal drugs.
Alcohol and drug abuse have an adverse affect on job performance, create dangerous situations and serve to undermine our customers and the community's confidence in our company. Our company cannot and will not condone drug or alcohol abuse on the part of its employees, nor will it condone any employee behavior on or off the job that may serve to damage the company's reputation.
Our policy concerning drug and alcohol use and abuse is as follows:
- The company will not hire anyone who is known to currently abuse substances.
- Employees must report to work in a fit condition to perform their duties. Being under the influence of drugs or alcohol is not acceptable.
- Any employee on company business, on or off company premises, is prohibited from purchasing, transferring, using, or possessing illicit drugs or alcohol or prescription drugs in any way that is illegal or counter to published policy.
- Employees will not be terminated for voluntarily seeking assistance for a substance abuse problem, however, performance, attendance, or behavioral problems may result in disciplinary actions, up to and including termination.
- Employees arrested for off the job illegal drug involvement will be considered to be in violation of the company substance abuse policy, and if convicted that will be grounds for automatic termination.
- Where available evidence warrants, the company will bring matters of illegal drug abuse to the attention of appropriate law enforcement authorities.
Types of payments
Physical therapy services will be billed to your participating insurance company. Patients with a co-insurance will be billed directly. (Definition of co-insurance: a percentage that is given to the patient from the insurance company for medical services. For example: If your plan covers 80%, the 20% is the patient's co-insurance).
Types of Payment Accepted:
- Cash
- Check
- VISA
- MASTERCARD
Copays
Co-payments are expected at the time of service. (Definition of co-payment: a designated amount assigned to the patient for medical treatment that is expected at the time of service. For Example: An insurance company covers 100% and gives the patient a $20 co-payment to pay each visit.)
